Request each record from the right issuer

Map each document to the organisation that holds the original record. Your school supplies education evidence; the licensing authority verifies professional registration; an employer supplies the requested employment or practice-hours evidence. A recruiter’s summary does not replace a required direct-source record.

You trained in India, work in Muscat and are applying to a Canadian province. Plan separate requests for education, Indian registration and any Oman records required by that province. The province decides which evidence and assessment route it accepts.

  • Identity: passport and name-change evidence if applicable
  • Education: qualification, transcript and any required theory/clinical details
  • Registration: current and previous licences requested by the destination
  • Employment: dates, role and hours when the receiving authority asks for them
  • Translations: follow the recipient’s accepted translator and delivery rules

Oman: the official verification route

Oman’s MOH health-practitioner registration service covers medical, nursing and other health professions for government and private institutions. The Directorate General of Private Health Establishments also publishes a work-verification and good-standing request form.

If your nursing record is held through a private establishment, check that directorate’s current form and submission instructions. If you worked in a government institution, ask the MOH department responsible for your record which nursing verification route applies. Do not use a pharmacy-specific certificate service simply because its title says good standing.

Ask whether the requested evidence verifies employment, professional registration or both. Your destination may require the issuing authority to send its own form directly. Confirm that delivery rule and the correct nursing service before arranging translations, courier delivery or payment.

A practical request sequence

Prepare the receiving authority’s current form before opening a local request. Use its application reference and official recipient, and check whether it accepts electronic delivery. Some authorities require records to come directly from the issuer; forwarding your own copy can leave the application incomplete.

  • Identify the MOH registration record and whether your employment was public or private.
  • For private-establishment records, check the directorate’s published work-verification form.
  • Ask the correct MOH nursing department about scope, recipient forms and direct delivery.
  • Keep employment evidence separate from registration and education evidence.
  • Retain the submission reference and verify receipt with the destination.

Check these details before sending

Match names and dates across the passport, school records and licence history. Keep evidence of an official name change rather than editing a document yourself. Ask the receiver how it wants discrepancies explained.

Record a sent date and a received date separately. A courier receipt or completed local request is not proof that the destination accepted the document. Ask for confirmation in the recipient’s application portal or through its official contact route.
