Request each record from the right issuer
Map each document to the organisation that holds the original record. Your school supplies education evidence; the licensing authority verifies professional registration; an employer supplies the requested employment or practice-hours evidence. A recruiter’s summary does not replace a required direct-source record.
You hold a DHP licence in Qatar, but your degree and first nursing registration came from the Philippines. A destination board can require both Philippine and Qatar records. The DHP electronic request addresses the Qatar registration part.
- Identity: passport and name-change evidence if applicable
- Education: qualification, transcript and any required theory/clinical details
- Registration: current and previous licences requested by the destination
- Employment: dates, role and hours when the receiving authority asks for them
- Translations: follow the recipient’s accepted translator and delivery rules

A practical request sequence
Prepare the receiving authority’s current form before opening a local request. Use its application reference and official recipient, and check whether it accepts electronic delivery. Some authorities require records to come directly from the issuer; forwarding your own copy can leave the application incomplete.

- Apply through the DHP electronic registration and licensing system.
- Attach the required destination verification form when applicable.
- Confirm the official recipient and retain the electronic request reference.

Check these details before sending
Match names and dates across the passport, school records and licence history. Keep evidence of an official name change rather than editing a document yourself. Ask the receiver how it wants discrepancies explained.
Record a sent date and a received date separately. A courier receipt or completed local request is not proof that the destination accepted the document. Ask for confirmation in the recipient’s application portal or through its official contact route.
