What a useful rationale should reveal

A useful explanation identifies the relevant clinical cue, connects it with the nursing decision and explains why alternative answers are less appropriate. Read every option, including those you did not choose. An answer key alone does not show the decision process.

Understand the scoring rule used in the example

The interactive exercise below demonstrates one plus/minus rule. Correct selected options add a point; incorrect selections subtract a point; the displayed item score has a zero floor. This is an educational example, not a complete model of every NCLEX item or the adaptive exam.

Other formats use other scoring rules. Do not apply minus-point advice to every question. The full partial-credit guide explains 0/1 and rationale rules with worked examples. Selecting every option is not a reliable substitute for analysing the clinical evidence.
