Prioritisation: which patient needs assessment first?
Original fictional learning example: you receive handover for four adults. A has long-standing arthritis with unchanged discomfort. B has diabetes with a scheduled teaching session and no new symptoms. C suddenly has one-sided weakness and trouble speaking. D has stable observations and is waiting for routine discharge teaching.
C needs immediate assessment and escalation under the facility’s stroke/emergency pathway. Sudden focal neurological changes are time-sensitive. Establish symptom onset or last-known-well information without delaying emergency action. This example teaches recognition and escalation, not a treatment order.
- A: chronic, unchanged symptoms do not outrank a new neurological change in this scenario.
- B: education matters, but there is no acute change described.
- C: new weakness and speech difficulty are concerning stroke signs.
- D: routine teaching can wait while the urgent change is addressed.

NGN: see how a selection can change the score
Fictional scoring demonstration: a select-all-that-apply item has three keyed options, A, C and E. Under a plus/minus rule, selecting A and C earns two points. Selecting A, B and C earns one point: two correct selections minus one incorrect selection. Selecting A with B and D gives zero after the negative total is floored at zero. Selecting A, C and E earns three.
This demonstration does not apply to every item format. The 0/1 rule awards points for correct response units without deducting points for incorrect units. Rationale rules require specified linked responses to be correct. Follow the instructions for the particular item, including whether a fixed number of choices is requested.
- Under plus/minus scoring, an unsupported extra selection can remove earned credit.
- Choose options because the case supports them, not because more selections feel safer.
- Item points are not a direct pass percentage or a complete description of CAT ability estimation.

From one mistake to a learning loop
Illustrative study pattern, not a student outcome: suppose you repeatedly miss questions involving a new change from baseline. Use the rationale to identify the missed cue, use Mistake DNA and analytics to find related patterns, then practise another targeted set.
For a physiology gap, open the relevant Floor Ready lesson. For a reasoning gap, ask the AI Tutor a precise question such as “Which new cue changed the priority?” Revisit the explanation and source rather than treating an AI reply as a clinical order.
